To keep staff, students, and visitors safe, we are following local health department recommendations and requiring that every participant in the district’s Academic Support Center program be assessed for COVID-19 symptoms and risk factors each day before entering the school site.

The survey below must be completed prior to your child’s arrival at the school site. If you do not complete your child’s screening prior to arriving, you will need to wait with them while the program’s nurse conducts an active screening.

Regardless of survey results, if you feel that your child has symptoms related to COVID-19 please contact a healthcare professional.
